The Live Alone Program

Facing a life-limiting illness is difficult, but the fear of facing it alone- or being in a strange place can be devastating. Traditionally, admission to hospice care has required that a family member, friend or hired staff fill the role of primary caregiver. When such individuals are not available to assist with the care of the patient, people are faced with the possibility of leaving their homes and relocating to unfamiliar surroundings or not having hospice care at all.

Those choices are usually not how the patient wants to receive care, or where the patient wants to live at this time in his or her life. Repeated studies and our own experiences have shown that people prefer dying in their own homes surrounded by their memories, family and friends.

Hospice of Green Country’s Live Alone Program is changing that situation. The Live Alone Program meets the needs of patients who lack adequate caregiver support.

Services begin with an assessment of the medical condition of each patient referred to Hospice of Green Country. When a patient is identified as a candidate for the program, a social work assessment determines the patient’s ability to perform activities of daily living; availability of any family, friends, neighbors, or church members to assist the patient; and the necessity of designating someone with durable power of attorney to take charge of the patient’s business affairs should it become necessary. Hospice of Green Country staff collect patient financial information to ensure the proper stewardship of our resources.

Eligibility

Admission into The Live Alone Program requires hospice patients to:

  • be a current HGC patient.
  • be psychologically stable
  • have adequate housing
  • be willing to complete a financial assessment to determine need.
  • Lack adequate caregiver support.

Services
Hospice of Green Country assists these patients in accessing special services that support their safety and quality of life. Some of these services include, but are not limited to:

  • Help with meal preparation, shopping and household chores
  • Frequent telephone contacts with Hospice of Green Country staff.
  • A medical alert system to assure the patient prompt emergency assistance
  • A door lockbox allowing Hospice of Green Country staff entry
  • Assistance in the process of assigning power of attorney
  • Volunteer and/or paid healthcare providers when Hospice of Green Country staff determine the patient is no longer safe alone.

Through this innovative and unique program, the full array of the specialized end-of-life care services offered by hospice is available to patients who have no or limited access to caregivers so they are able to remain in their homes as long as possible.
